Who needs a Summary of Chancellor's Regulation?
This is a questionnaire for teaching professionals who plan on participating in a per session program. Future per session employees have to file this application also if they plan to spend more than 500 hours in class during their course.
What is the Summary of Chancellor's Regulation for?
This is one of the basic documents in university employment. It was designed according to standard Operating Procedures Manual for Schools. This summary, also known as Form C-175, is requested in the State of New York.
Is it accompanied by other forms?
Complete at least two copies, so the per session supervisor can keep one for his records. Other addenda are not necessary.
When is Summary of Chancellor's Regulation due?
The completed form should be filed with the per session supervisor prior to commencement of employment in a per session activity.
How do I fill out a Summary of Chancellor's Regulation?
Write your full name, address and a phone number. Check the box if you are a full-time employee of NYC Department of Education and if the answer is “Yes”, indicate your current location, title and hours of employment. Describe the per session position you’re applying for. Give the program name, approximate start date, school and total hours of activity. Check the box if you plan to work in any other per session activity during the same period for which you are applying. Also, inform a supervisor if you have submitted a waiver request to exceed a 500-hour maximum.
